Self-determination is a national movement to redesign the delivery of services and supports to individuals with developmental disabilities.  The self-determination philosophy is a belief that one should have choice and control over all aspects of life.

Services include concepts such as person-centered planning and individualized budgeting, funding following the individual and the provision of self-directed supports to individuals and their representatives.

    There are four principles of Self-Determination.  

 
  • Individuals have the freedom to plan their own lives. This includes having choice and control over one’s own individual planning process, and in particular, decisions affecting the nature of the services and supports he/she receives and how and by whom they are provided.

  • Individuals have authority or control over their own lives. This includes having control over resources including the authority to make decisions regarding the development of an individual budget that funds the implementation of the individual’s service plan.

  • Individuals have access to the support they need. This support should enable individuals to personally manage the services they receive, to make informed choices based on information about the options available, including customized services, and to have the opportunity for increased community integration.

  • Individuals take on the responsibility of living in interdependent communities.

(Nerney and Shumway, 1996; Moseley, 2002)

The unifying force is that individuals and their families/representatives have the primary authority to make choices that work best for them, regardless of the nature and/or extent their disability, functional limitation(s) or source of payment.   Services:   Roles and Responsibilities of ISO:  
  Services provided by Futures include, but are not limited to:   Futures is the designated agency providing support services to the individuals/representatives in a self-directed manner. The agency is the common law employer of the qualified support service workers. As the common law employer of individuals’ support service workers, an Agency with Choice ISO is responsible for:
  • Invoicing the government agency for services rendered.

  • Processing employment documents.

  • Conducting criminal background checks on prospective workers.

  • Preparing and disbursing payroll checks.

  • Conducting worker training with the individual/representative.

  • Providing a variety of support services to individuals /representatives (i.e., skills training, developing and maintaining a worker registry, conducting individual needs assessments.

  • Monitoring quality of services provided and individual/representative satisfaction.

  • Administering payments to vendors for one-time only services purchased for individuals including, but not limited to environmental adaptations.

Roles and Responsibility of Individuals/Representatives of Using Agency with Choice ISO's:

The individual/representative acts as the managing employer of the qualified support service workers he/she recruits and refers them to Futures for employment and assignment back to the individual/representative. As the managing employer, individuals/representatives are responsible for:

  • Recruiting/selecting and referring their qualified support service workers to the agency for hire and assignment back to the individuals.

  • Providing training to their qualified support service workers.

  • Determining their qualified support service workers’ work schedules, the tasks they are to perform, and how they are performed.

  • Supervising the day-to-day activities of their qualified support service workers.

  • Discharging their qualified support service workers, as necessary, from their homes.

  • It is important to understand that individual/representative and Futures will work as a team in managing qualified support service workers. However, the individual should take the lead in determining the who, what, when and where regarding the provision of his/her supports.

Qualifications and Responsibilities of a Support Service Worker:

  • Must be least 18 years of age.

  • Must have a criminal history background and child abuse clearance check.

  • Must be able to respond to the requests of the individual /representative (i.e. scheduling, experience, flexibility).

  • Must have valid driver's license.

  • Must complete Futures' orientation and preliminary in-service training according to Futures' personnel policies.

  • Complete all documentation as required by Futures, Bradford County Human Services and the and individual/representative.

  • Follow Futures' personnel policies.

Individual's Bill of Rights:

   
   
Considerate and Respectful Care - You have the right to be treated with dignity and respect at all times by all support service workers and providers.

Confidentiality in Care - You have the right to know that the care you receive and any information about your household will be kept private.

Have Time to Yourself - You have the right to say when your workers and others will be in your space and where they can go in your space.

Set Your Own House Rules - You have the right to set rules and regulations for the people who come in and work for you. For example, you have the right not to allow workers to smoke or make personal phone calls.

Refuse Treatment - You have the right to refuse to accept any of the treatments that are provided to you.

Free from Mental or Physical Abuse - You have the right to be free from mental abuse including: Humiliation, harassment and threats of punishment or deprivation. You have the right to be free from physical abuse including: corporal punishment and the use of restraints as punishment.

Change Your Care Provider - You have the right to terminate your support service worker or provider and replace them with a person or provider who is more to your liking, within the guidelines of federal and state labor laws pertaining to hiring and termination of workers.

Make Your Own Decisions - You have the right to make your own decisions about your personal care and what you choose to receive at any time.

Know of Any Changes or Amendments to Your Services or Programs in a Timely Manner - You have the right to be informed of any changes or amendments to your services or programs within a reasonable time in order to be able to react to them accordingly.

To Leave the Program and Return to Other Care Providers - You have the right to discontinue participation with the ISO and return to other care providers at any time.
