The homeless assistance program
(HAP) makes
available a continuum of services to homeless and near homeless
individuals and families living in Bradford and Sullivan Counties in
North Central Pennsylvania. HAP funding is provided by the Department of
Public Welfare, Office of Social Programs.

Rental Assistance This component provides payments for rent, rental costs for trailers and trailer lots, and security deposits to prevent and/or end homelessness or near homelessness by maintaining individuals and families in their own residences. Emphasis is placed on the prevention of homelessness for families with children. Emergency Shelter This component is defined as a refuge and care to persons who are in immediate need and are homeless; and have no permanent legal residence of their own. Services include mass or individual shelter in congregate settings, and individual shelter paid through a voucher system to house clients in hotels or motels. When an individual is placed in a hotel or motel, Futures cannot pay for meals separately. Meals are eligible expenses only if they are included as part of the billing or per diem charged for the overnight stay.

This component coordinates with the individual to achieve the goal of self-sufficient living. Case Management begins with the intake process and assessment and includes setting goals in the area of basic life skills such as:
Case Management services must be made available to anyone receiving services in the Homeless Assistance Program. Individuals at risk of becoming homeless in the future are also targeted for Case Management services. Case Management services are required when an individual requests Homeless Assistance services a second time. If a second time applicant refuses Case Management services, the applicant may automatically be denied. |

Eligibility
Be at 150% poverty level. (People escaping domestic violence do not need to meet the income guidelines.) Have adequate income to pay rent/utilities in subsequent months. Provide the other half of the requested amount. Not to exceed the maximum amounts for family ($750.00) and individual ($500.00) in a 12 month period. Are willing to accept case management services if a second time user.

GUIDELINES Futures will only assist with 50% of the required amount within the maximum limits. The applicant will be responsible for the other 50%, either through a payment plan with the landlord or utility company, the assistance of another agency, help from a family member or by their own contribution. Any requests from a second time applicant will require their receiving budget assistance through case management. Refusal of case management may result in a denial. All required paperwork will be the responsibility of the applicant and in place before a review for approval. Proof of 100% of the amount needed must be verified before a check is issued. Checks will be issued once a week except in the case of an emergency. All payments made from the HAP fund are issued directly to the landlord or utility company. No payments are made directly to the applicant.
